Position Overview

Intercambios Puerto Rico is a non-profit community-based organization that promotes the social integration of marginalized groups, including drug users, homeless, and sex workers. We work from a scientific perspective of Harm Reduction through service provision, education, advocacy, and research. We envision a Puerto Rico where marginalized populations have access to all opportunities for individual and collective development that makes it possible for them to attain a decent life and contribute to society. Intercambios Puerto Rico is an affiliate of Housing Works, Inc., the nation’s largest community-based AIDS service organization as well as the nation’s largest minority-controlled AIDS organization. Position Specifics: • Full time position - 40 Hours a week • Great benefits • HRSA approved loan repayment site • Remuneration commensurate to experience.

Responsibilities

Responsibilities:• Provide services to individuals (and families) who are experiencing homelessness, substance use, and/or co-occurring mental health disorders directly in the community. • Screen individuals (mostly in the community) for recruitment to the GBHI Project. • Complete GPRA • Carry out the processes in compliance with the rules, procedures and protocols of the organization. • Assess needs of program participants, create treatment plans and follow up. • Coordinate participant transportation to services. • Link program participants to care and support services via referrals and follow up. • Link participants to housing via referrals and follow up. • Referred and coordinate services with internal and external programs for participants in need. • Document and update the participant record according to the time set. • Provide follow up services to program participants according to the time set. • Represent project and agency in different forums • Coordinate with other team members for participant services or support • Visit organizations and services providers to create collaboration relations for the benefit of the GBHI Project.

Minimum Requirements

Educational Requirements

Experience and Requirements:• Experience with providing case management services to people that have experienced homelessness, substance use, and/or co-occurring disorders • Knowledge about HIV/AIDS, Hepatitis-C and STDs in the Puerto Rican context • Knowledge and/or experience with Evidence Based Practices / Interventions • Highly motivated to work with people who experience homelessness, substance use, and/or co-occurring disorders in an outreach setting • Capable of executing a plan based on facts and scientific knowledge • Proficient and efficient with multiple computer software programs • Meticulous at maintaining records and confidentiality protocols • Good written and verbal communications skills (in both Spanish and English a plus) • Outreach and participant navigation experience • Able to work closely with others in outreach team and office setting (team player) • Up to date driver’s license • Bachelor of Arts in Social Work or equivalent.
