The main objective is to complete tasks associated with the patient care plan that supports health plan quality measures. These quality measures are often associated with physician payment incentives, and the Care Coordinator is an important part of the overall financial performance of the practice. The Care Coordinator works with the patient, the PCP, the family/caregiver(s), and third party care providers.Essential TasksSuccessfully completes and documents daily care coordination tasks including: patient communications, scheduling, order entries, results gathering, etc.Maintains fluency in the care plan needs of the PCP’s patients.Proficiency in the care planning software application.Assures a positive working relationship with the PCP and office staff.Maintains a positive relationship referring to providers.Uses tenacity and creativity to close gaps in care plans.Ensures timely communication with members of the care team in order to close gaps in care.Reviews medical records to identify required documentation to close gaps in care.Provides feedback for the improvement of the Care Coordination Program.Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality always, and follows HIPAA guidelines and regulations.Respects patients by recognizing their rights; maintaining confidentiality.Attendance to healthcare industry conventions, seminar or other related activity.Committed to simplicity and maximizing value according to lean processes.Requirements:Bachelor’s Degree in Nursing with RN License with two years of related experience, or Associate's degree with RN License and five years of related experience or Clinical Social Worker (MS) with RN License with two years of experience.Two (2) to five years of experience or more (depending on education) in the Health Industry, including billing processes, knowledge of medical office operations, electronic medical records or related services.Knowledge in classification disease systems and procedures.Knowledge in applicable state and federal regulations of health industry.Critical thinker; analytical skills.Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal.Positive attitude and respectful, professional customer service.Organized and excellent time management skills.Strong MS Excel, Word, and Power Point knowledge.Bilingual in English and Spanish.Leadership, communication, education, and counseling skills.Proficiency in communication technologies (email, cell phone, etc.)Inmediata Health Integrated Solutions is an Equal Employment Opportunity Employer.
