Descripción del Empleo:
We are a clinical service provider in Puerto Rico specializing in Complex Care Management. Office Hours Monday to Friday 8am to 5pm. We have internal medicine, nephrology, endocrinology and another medical professional.
Requisitos:
Education - Bachelor’s degree (BSN) or Master is preferred.
Good Standing and other nursing licensing requirement.
Experience and Required Skills:
At least 3 years of previous experience in management of chronic diseases patients and case management experiences is required.
Advance knowledge in renal disease, a combination of hemodialysis, PD CKD education training highly valued.
Excellent written and verbal communication skills – good presentation skills.
Ability to communicate and maintain effective interpersonal relations at various levels of the organization.
Bilingual (Spanish – English)
Knowledge of electronic medical record and computer skills.
